Job Overview:

The Community Paramedic is responsible to follow-up with a patient within the 48-hours after hospital discharge to ensure that patient receives adequate follow-up assessment and care so that preventable causes of readmission are addressed and mitigated.

Primary Scope of Responsibilities:
  • Participate as part of a community based team to provide integrated out-of-hospital medical services to address a consumer s particular non-urgent medical condition
  • Provide information to CIHCS Agency consumers about relevant local community resources and other collaborative services
  • Examines, screens, treats and coordinates health services for patients
  • Conducts post-hospital discharge follow up care, including but not limited to, monitoring medication, dressing changes, and monitoring chronic disease processes and vital signs
  • Observes, records and reports to physician as to patients conditions and reaction to drugs, treatments, and/or significant events
  • Conducts patient education, including diabetes prevention and treatment, hypertension, Congestive Heart Failure (CHF), chronic obstructive Pulmonary Disease (COPD), fall assessment, injury evaluation, geriatric frailty visits and nutrition
  • Coordinates appointments and follow up with physicians and hospital departments
  • Administers patient care consistent with protocols and physician orders
  • Develops and completes appropriate Patient Care Reports
  • Attends pertinent meetings as requested and available
  • Provides and participates in training to maintain higher competencies of Community Paramedic
  • Complies with hospital infection control, disaster and emergency procedures
  • Performs other related functions as necessary
  • Responsible to follow-up with a patient within the 48-hours after hospital discharge to ensure that preventable causes of readmission are identified and mitigated.
  • Adheres reliably to all accepted patient safety standards.
  • Coordinating Community Paramedic related patient services
  • Performs patient medication reconciliation to check for compliance according to discharge medication orders.
  • Serves in advocacy role; encouraging active involvement of patient and family in their care
  • Prioritizes patient s situations and takes appropriate action in an efficient, effective manner.
  • Makes value-added resource decision for patient care with respect to supplies, equipment and acuity of care
  • Prepares clinical notes and documentation and complies with the Aspen Valley Hospital Reporting Requirements (**List reference number**)
  • Communicating appropriate medical status and treatment information to the patient and/or designated representative and, as applicable, the patient s care provider.
  • Keeps patients informed regarding what to expect pre/during/post procedure or assessment
  • Provides individualized patient and family education consistent with patient s learning style, readiness to learn and self-care needs
  • Evaluated the effectiveness of patient learning
  • Promote consumer dignity, independence, self-determination, privacy, choice and rights
  • Provide resources available to the patient within the community that may help provide for the patient needs and to prevent injury, illness, abuse and neglect
  • Provides oversight of care as needed of a Paramedic or EMT assistant
  • Operates Emergency vehicles and equipment safely and effectively
  • Maintains accurate and timely medical reports, charges, and logs including: ambulance report, ambulance run log, and vehicle log
  • Performs functions of a medical/nursing assistance nature at the direction of physicians in assigned areas within the certified capability of the Community Paramedic
  • Attends any additional assessment or procedure training as required by the referring physician
  • Maintains any associated equipment in constant state of readiness to include cleaning, inventory and maintenance checks as per the appropriate check-off lists
  • Assists with maintenance and cleaning of Aspen Ambulance District building, quarters, and bays
  • Assists in Department planning and offers suggestions for improvement of operations and equipment
  • Orients students or new EMTs to Ambulance and Hospital Emergency Unit Operations. Assists in coordination of continuing education and recertification of Hospital employed EMTs and nurses when possible
  • Ability and skill to perform basic life support as defined by Colorado State Rule 500/800 and applicable waivers
  • Demonstrates the ability to assess and interact with customers of all ages, recognizing language and learning barriers, and technology challenges.
  • Able to state his/her duties during a code pink drill.

Minimum Qualifications:
  • Must have completed approved Community Paramedic course: affiliated with a community college or university, completed required course didactic and clinical requirements
  • Has successfully passed the International Board of Specialty Certifications CP-C Exam
  • Ability to read and communicate effectively in English
  • Bi-Lingual in Spanish highly desired
  • In depth and up-to-date skills and knowledge of pre-hospital Emergency Medical Care techniques at the Paramedic level in order to provide emergency treatment for injured as normally acquired through the successful completion of a Colorado Department of Health Emergency Medical Technician Paramedic Certification and current ALS Certifications
  • Written, verbal, quantitative and interpersonal skills normally acquired through the completion of a High School Degree in order to complete medical records, communicate effectively with patients and other Emergency Responders and to interrelate appropriately with Hospital personnel.
  • Desired, but not mandatory, the ability to evacuate and treat patients in the mountain and steep off-road settings normally acquired through completion of an off-road mountain evacuation course.
  • Ability to concentrate and show attention to detail
  • Relatively high degree of analytical abilities
  • Strong interpersonal skills required
  • Ability to work independently
Education/Certification Requirements:
  • Colorado State Community Paramedic endorsement required
  • Current Colorado State EMT-P certification required
  • National Registry EMT-P certification preferred
  • BLS certification required
  • ACLS certification required
  • PALS or PEPP certification required within 1 year of hire
  • NRP preferred
  • PHTLS or equivalent preferred
  • Current Colorado Driver s License required
Working Conditions:
